Dental Insurance

Your dental plan is a form of compensation provided by your employer and/or yourself. You can expect your Insurance Company to reimburse a portion of our fees. That portion is determined by the contract between you, your employer and the insurance company. Generally, the higher the premium paid by you or your employer, the more generous the reimbursement.

Although we are not a party to the contractual arrangement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled. As a convenience to you, we will process your insurance claims in order for you to receive this maximum benefit. We will also gladly provide dental x-rays and a written diagnostic narrative report should your insurance company have any questions about the services provided.

Unfortunately, there are limitations to dental treatment imposed by many insurance company contracts. It is our ethical duty to offer you all your options for dental care. To do otherwise would violate our contract with you – a contract we feel morally obliged to honor.

Payment Options

For your convenience, we accept cash, personal check, debit card, Visa, MasterCard, and Discover. Our goal is to deliver the finest care at the most reasonable cost to our patients. Therefore, we offer a 5% discount on treatment plans paid at the time of service by cash or personal check. Payment is due at the time service is rendered unless other arrangements have been made in advance. If you have concerns regarding your account, please contact us at (262) 692-2461. Many times, a simple telephone call will clear up any questions.

If you have dental insurance we will process your claims and accept assignment of benefits. Most insurance companies will respond within 4 - 6 weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. Please remember you are fully responsible for all treatment fees incurred regardless if you have insurance coverage or not. We can make arrangements for a monthly payment plan but this should be accomplished prior to the actual procedure.
