To request an appointment, please fill out the form below. Our scheduling Coordinator will contact you in regards of the appointment requested as well as guide you with all the new office protocols for the prevention of COVID-19.

COVID-19 OFFICE PROTOCOLS:

1. Must wear a mask until assistant or doctors request for removal.

2. If you are experiencing any COVID-19 symptoms, inform our front desk in order for your appointment to be rescheduled. 

3. Use hand sanitizer as needed.

4. We are required to scan body temperature. 

We utilized additional equipment on the clinical area; such as, air purifiers, aerosol vacuums and Isolates for the prevention of COVID-19.

At Millennium Dental we strive to acquire the latest technology in dentistry in order to keep providing excellent service. We believe having an excellent 3D X-ray machine will allow us to go beyond the average level of dentistry. 

If you are a dental provider and need to refer a patient for a CT scan, gladly we can help. Fill out the referral form, print and remind the patient to call us and schedule an appointment, referral form is required.
