Your initial appointment will be a scheduled consultation where we take pride in first listening to your concerns before explaining your diagnosis and treatment options. You see, this we realize this is something you have probably been thinking about or living with for along time. While you may have pondered over moving forward doing so was a truly tough task. That is why we want to know you and why creating a new smile or enhancing your already brilliant smile is important to you. Occasionally, treatment can be performed on the same day as the consultation. However, a complex medical history or treatment plan will require complete evaluation and a second appointment to provide treatment on another scheduled day.
At the time of consultation, please furnish the following documents:
- X-rays (if applicable)
- A list of your current medications
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
Please fill out the following forms before your initial visit
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), or if you are on any medication (i.e. heart medication, aspirin, anticoagulant therapy, etc.).
NOTE: A parent or guardian must accompany all patients under the age of 18 to a consultation.
